Claims

To start your claim request, please download the MaxiLife Claim Form for Individual Products. Complete all the required fields, then print the form and affix your wet signature. Once you have signed the form, scan it along with required documentation and send it via email to our Claims Department at claims@maxilife.com.ph

Electronic copies of your claim documents will suffice.  Please ensure that all documents are complete, clear, legible and that scanned copies are of good quality to facilitate easy processing by our Claims Department.

Yes, you can submit foreign medical records and death certificates as evidence for your insurance claim. However, these documents must be authenticated by the Philippine Consular Office or apostilled in the country where they were issued. This ensures their validity and authenticity.

We accept electronic submissions, so yes you can file claims while you are overseas. However, the Claimant’s Statement Form and Special Power of Attorney you will issue needs to be authenticated by the Philippine Consular or apostilled in the country you are currently in at the time of filing.

Yes, someone else can file a claim on your behalf provided they are your authorized or legal representative.

The claims process generally takes 10 business days once we receive all the necessary documentation.  If additional information or further investigation is required, the claims process may extend to 25 business days from completion of all required documentation.

We will still accept and process your claim submission, even if it is made three (3) months after the event. While we encourage timely submission for efficient processing, submitting your claim after this period will not invalidate nor reduce the claim.

Your claim benefit will be credited to the bank account you nominated in the MaxiLife Claim Form.

TriShield9 Life

TriShield 9 Life is a non-participating individual life insurance plan that provides living and death benefits.

For an affordable premium, customers can pay for 3 years for a 9-year coverage.

Customers can be assured of funds in case of emergency. This is the living benefit which can be withdrawn from the policy’s cash values. 

Beneficiaries will receive the death benefit which is equivalent to the sum assured if the Insured dies within the covered term.

The plan is bundled with Accidental Death Benefit and Waiver of Premiums supplementary riders, forming a package that offers more comprehensive protection.

  • Accidental Death Benefit Rider -if the insured dies from an accident, a benefit equal to the rider sum assured indicated in the policy contract is paid to the beneficiary/ies. Accidental death means death within one hundred eighty (180) days from the date of the Accident.
  • Waiver of Premium (due to disability) Rider - if the insured becomes totally and permanently disabled, all future premiums from the time of disability, will be waived and considered paid, subject to the conditions specified in the rider contract. 

TriShield 9 Life may be purchased by any individual who meets the following minimum and maximum entry ages:

TriShield 9 Life provides coverage for 9 years and provides the following product features:

1. Insurance Coverage death benefit equal to the Sum Assured less any outstanding policy loans and interest will be payable in the event of death of the Life Insured.

2. Accumulation of cash values – amount of cash value depends on Sum Assured of the policy, the Insured’s issue age,  the length of time the policy has been in force, and the length of the policy’s premium payment period. Policy loans may also be applied from the cash values in case of emergency.

3. Non-Forfeiture Options – allows the policy to be in force when premium due is not paid and the policy has remaining cash value. Customers may choose from these options – Cash Surrender Value, Reduced Paid Up, or Automatic Premium Loan. 

If no option is elected and if a premium due remains unpaid at the end of the grace period, the plan shall automatically continue in force as non-participating Reduced Paid-Up insurance.

The following are the available non-forfeiture options for TriShield 9 Life

  1. Reduced Paid-Up – If you fail to pay your Premium by the end of the 31-day grace period but your Policy has a Cash Surrender Value and you have chosen the Reduced Paid-Up Insurance as your Non-Forfeiture Option, your Policy will continue until its Expiry Date without you paying further Premiums but the Amount of Insurance will be reduced and the coverage provided by rider benefits shall be terminated.
  2. Automatic Premium Loan – if you have chosen this option, any premium due remaining unpaid at the end of the 31-day grace period is automatically deducted from your Total Cash Surrender Values, subject to the terms and conditions of the Policy.  .
  3. Cash Surrender Value - while the Policy is active and not under any Non-Forfeiture options, the Policy may be surrendered for its Cash Surrender Value derived from the Table of Cash Surrender Values. The appropriate cash values can be read off from the table based on the issue age and the policy year the cash value will be availed. When your Policy is surrendered for cash, the Policy automatically terminates and the Cash Surrender Value of this plan, if available, is paid to the Policy Owner.

TriShield 9 Life is available through various product distribution channels of MaxiLife. Should you need higher coverage not in the online packages, our insurance agent can provide assistance and guide you on your journey.

Customers can avail more than one TriShield 9 Life policy subject to MaxiLife’s underwriting / aggregation limit.

Automatic termination on TriShield 9 Life will effect on the earliest of the following:

  • Expiry Date, as stated in your Policy Details Page;
  • Death of the Insured; or
  • Non-payment of Premium after the thirty-one (31) days grace period. This is subject to the Non-forfeiture Option.
  • The date on which you surrender your Policy for its cash surrender value or the request to cancel your Policy.
  • When the accumulated cash has been exhausted due to Policy Loan.

TriShield9 Health

TriShield 9 Health is a non-participating individual life insurance plan that provides living and death benefits.

Customers can be assured of funds in case of emergency. This is the living benefit which can be withdrawn from the policy’s cash values 

Beneficiaries (or the Insured in case of critical Illness diagnosis) will receive the  benefit which is equivalent to the sum assured indicated in the Policy in the event of death or critical Illness diagnosis, whichever comes first, of the Insured during the effectivity of the policy. The list of covered Critical Illnesses are listed in the Policy Contract. Upon claim of either the Critical Illness Benefit or the Death Benefit, the Policy will be terminated.

The plan is bundled with Accidental Death, Disablement, and Dismemberment and Daily Hospital Cash supplementary riders, forming a package that offers more comprehensive protection.

  • Accidental Death, Disablement and Dismemberment Rider - provides the payment of sum assured in case the insured dies, suffers disablement or dismemberment, due to accident. The sum assured to be paid will be in accordance with the Disablement Table and conditions specified in the rider contract.
  • Daily Hospital Cash Rider - provides hospitalization benefits - Daily Hospital Cash, In-patient Surgery Cash and ICU Cash Benefit in case the Insured is admitted as inpatient in a hospital, undergoes surgery or confined in ICU, subject to the limitations and conditions of the Policy.

TriShield 9 Health provides coverage for 9 years and provides the following product features:

  1. Insurance Coverage we will pay you or your Beneficiary, whichever is applicable, the Amount of Insurance, as provided in the Policy Details Page, in the event that the Insured either dies or is diagnosed with a covered Critical Illness during the effectivity of this Policy, whichever occurs first subject to the provisions herewith. Any Indebtedness under this Policy will be deducted from such payable amount.
  2. Accumulation of cash values – amount of cash value depends on Sum Assured of the policy, the length of time the policy has been in force, and the length of the policy’s premium payment period. Cash values may be left to accumulate and be obtained due to early policy surrender. Policy loan may also be applied from the cash values in case of emergency.
  3. Non-Forfeiture Options – allows the policy to be in force when premium due is not paid and the policy has remaining cash value. Customers may choose from these options – Cash Surrender Value, Reduced Paid Up or Automatic Premium Loan.

The following are the available non-forfeiture options for TriShield 9 Health

  1. Reduced Paid-Up – If you fail to pay your Premium by the end of the 31-day grace period but your Policy has a Cash Surrender Value and you have chosen the Reduced Paid-Up Insurance as your Non-Forfeiture Option, your Policy will continue until its Expiry Date without you paying further Premiums but the Amount of Insurance will be reduced.

The appropriate cash values can be read off from the table based on the issue age and the policy year the cash value will be availed. When your Policy is surrendered for cash, the Policy automatically terminates and the Cash Surrender Value of this plan, if available, is paid to the Policy Owner.

  • A written notice claim within thirty (30) days from critical illness diagnosis or death, whichever is applicable;
  • A written proof of claim can be submitted to claims@maxilife.com.ph or at our Head Office and other duly authorized offices within ninety (90) days after the date of Critical Illness diagnosis or death, whichever is applicable.

Note: Failure to submit within the given periods will not invalidate nor reduce the claim IF the beneficiary/ies or the Insured’s authorized representative can show that it is not reasonably possible to submit such notice or proof within the required periods.

HealthEase

Health Ease is an annual-renewable accelerated Critical Illness Term Plan.

The plan is renewable every year until the maximum renewable age provided in the contract.

Beneficiaries will receive the amount of insurance upon death of the Insured or upon diagnosis of any of the five (5) covered critical illness within the covered policy year, whichever occurs first.

HealthEase may be purchased by any individual who meets the following minimum and maximum entry ages:

Payment Term: Annual

Issue ages: 18 – 60 years old

HealthEase provides coverage for 1 year and provides death benefit equal to the Sum Assured upon diagnosis of any of the five major Critical Illness, specified in your policy contract, or upon death of the Insured, whichever comes earlier. Payment of the Sum Assured terminates the policy.

Non-forfeiture option is not applicable for this plan.

HealthEase is available through various product distribution channels of MaxiLife. Should you need higher coverage not in the online packages, our insurance agent can provide assistance and guide you on your journey.
For this product, each insured individual can hold multiple policies, with a cumulative limit of one (1) million pesos (Php) in insurance coverage at any given time.

Automatic termination HealthEase will effect on the earliest of the following:

  • Expiry Date, as stated in your Policy Details Page;
  • Once the death benefit or the Critical Illness benefit has been claimed;
  • Non-payment of Premium after the thirty-one (31) days grace period
  • The date on which we approve your request to cancel this policy.

Upon Death or Critical Illness diagnosis of the Insured, the beneficiary/ies (for death claim) or the Insured’s authorized representative (for critical illness claim) must submit to us:

Policies

You can purchase any of our plans if you are between the age of 18 to 60 years old, a Filipino citizen and a resident of the Philippines. 

You must be in the Philippines, at the time of application. 

You can pay in the MaxiLife Shop using your eWallet

Your insurance coverage will become active once you've paid the first premium and we've emailed you the policy contract.

A beneficiary is the person who receives the life insurance payout when the insured passes away. Common beneficiaries you may nominate are immediate family members.

Beneficiary designations can be revocable or irrevocable.

A revocable beneficiary assignment means that the Policy Owner can/has:

  • Change beneficiaries; and
  • Complete control over all rights and privileges on the policy without the consent of the designated beneficiary/ies

An irrevocable beneficiary assignment means that the Policy Owner cannot/does not:

  • Change beneficiaries; and
  • Have complete control over all the rights or privileges of the policy as the consent of the beneficiaries must be always secured. Therefore, all policy changes will have to bear the signature of the irrevocable beneficiary/ies (must be of legal age).

The death benefit will be paid to the insured’s estate.

You may choose from annual, semi-annual, quarterly or monthly payment modes.

If you missed paying during your due date, you have a grace period of thirty-one (31) days to pay your premium and maintain your coverage.  

If the payment is not made within this period, your policy will lapse, and your benefits will be terminated.

If your policy lapses due to non-payment, you may request reinstatement within three (3) years, subject to the following:

  • Completely filled-out Reinstatement Form
  • Payment of all back premiums with interest
  • Payment or resumption of any outstanding debt

Reinstatement is not possible if the policy has been surrendered for cash. The reinstated policy will only cover events occurring after the reinstatement approval date, subject to the incontestability provision.

Download the Reinstatement Form, complete the required fields, print and sign with a wet signature. Take a photo or scan the form and submit it to customercare@maxilife.com.ph. 

You may change your beneficiary/ies at any time after the policy has been issued. Please note that the most recent change will override any previously nominated beneficiaries.

Here are the steps and requirements in changing your beneficiary/ies:

  1. Download the Beneficiary Change Form.
  2. Complete and sign the form: Fill out all the required fields, print the form, and sign it with a wet signature.
  3. Complete the signatures of irrevocable beneficiary/ies (if applicable): If you designated the current beneficiary/ies as irrevocable and they are of legal age, you will need to obtain their written consent on the form as well. 
  4. Submit the form: Scan the signed form and email it to customercare@maxilife.com.ph.

Important Note: To establish insurable interest and relationship of the beneficiary to the insured, the following requirements must be submitted as supporting documentation to the change in beneficiary:

  • Birth certificate of the beneficiary
  • Marriage Contract, if beneficiary to be added is a married child of the owner
  • Letter establishing insurable interest, if beneficiary to be added is only a dependent of the insured and is not immediate family
  • Photocopy of two (2) valid identification cards

Here’s how to update your contact information:

  1. Download the Policy Details Change Form. 
  2. Complete and sign the form: Fill out all the required fields, print the form, and sign it with a wet signature.
  3. Complete the signatures of irrevocable beneficiary/ies (if applicable): If you designated the current beneficiary(ies) as irrevocable and they are of legal age, you will need to obtain their written consent on the form as well.
  4. Submit the form: Scan the signed form and email it to customercare@maxilife.com.ph

Within Cooling-Off Period

Your policy comes with a 15-day Cooling-Off Period.  Within 15 days from receiving the policy document, you may cancel your policy if you believe it is not suitable for your needs. If you cancel within this period, the insurance contract will be terminated, and you will receive a full refund of any premiums paid.

Beyond Cooling-Off Period

Beyond the 15-day cooling-off period, you may surrender your active policy for its cash surrender value. The value is determined based on your age when you purchased the policy and the year you surrender it.

Please note that surrendering your policy will terminate your coverage. Any outstanding loans or charges will be deducted from the cash surrender value before you receive any payment.
